Tennessee will be the Headquarters for the Fountain Center.

Introduction:

Vision– is to create a customer service center centered around the genuine needs of the individual and families in the community.

Mission– Our mission is to assist individuals or families with short-term assistance in the midst of an emergency or economic, financial hardship.

Target audience/populations: N, S, E, W, individuals, and family who need aid due to economic-financial hardship

Eligibility/Ethnicity: Legal US Residents/ All race.

**Emergency Assistance Program**

Priority assistance for elderly and mothers with children under 15, a medical life-threatening emergency.

Who can apply? Individuals or families

What are the criteria for assistance? Serious emergency needs, cutoff notices,

How long is the process? 1-3 days except for emergency circumstances only dedicate to the elderly or parents with infants or kids under five years old. We would assist these cases the same day before cut/off.

Plan, Priorities, and Budget. VOTM, long-term success is to establish a comprehensive framework that will comprise of our goals to be met with a four-to-five year period.

VOTM and The Fountain center want to see assistance provide to emergency needs quickly. We do not want to see individuals or families lose their residents. Providing assistance to those in need is our primary objective. TN will be our primary location and our goal is to open various locations.

We are depending on Angels Our goal would be to increase the number of investors by the end of the fiscal year. We will implore the SMART formula to aid us in achieving our long-term goals. We will be Specific as to how to execute our plan, ensure our goals are Measurable, Achievable/Attainable is crucial to our success.

The Fountain Center is very much Relevant/ Realistic from personal experience, testimony, and COVI9. The Fountain Center time-based will start Fall 2021, this is the purpose of participating in the BIGPayback.

Short-term Success

We will evaluate our progress in the present and create an action plan to improve performance daily. The short-term goals will be a segway to our long-term goals. We have set a timeframe of six months to have at least 50 investors or stakeholders collaborating with us on this mission. Through promoting our events, email blast, and social media posting. We are hopeful to meet our six-month short-term success goals.

As the Founder of VOTM, my dream is to open an emergency financial center for the elderly, individuals, and families that have fallen on hard times to due economic hardship. Our goal is to have a stand along build built within the next five years. The center will have a career corner, residential search corner, simple tax preparation corner.

Program Success Monitored By

We will monitor our program by our target dates; for example, we are striving to recruit 50 investors or stakeholders within six months. Additionally, there will be a process evaluation that will occur once the program implementation has begun, and it measures how effective the program’s procedures are. Afterward, we will collect the generated data to identify any inefficiencies and streamlining processes to external parties.
